I believe they are quite literally extra lives if you are in a hardcore world so you can respawn

If not playing in hardcore (perma-death) mode then you can save them until you unlock the repeatable quest to trade them for additional heart containers (unlocked after a few wither kills)

This is for the BQ hardcore mode; there is another repeatable quest that allows you to trade 4 of them in for a heart canister.
